It is a pretty simple questionnaire, and you can download the free printable here. {I printed mine on yellow paper, but you could use any color you like.}
My seven year old will fill it out by himself, and I will help the younger boys. I can't wait to see what they come up with. I plan on having them answer the same questions each year, and maybe I will make a book for each of my boys when they are older. :-)
